What We Do
Company Name is a Program Approved Service Agency (PASA) that takes individuals with developmental disabilities (IDD) from institutional environments, such as nursing homes, to a community-based environment by utilizing Home and Community-Based Services Waivers (HCBS).
The HCBS waiver is an extra set of Health First Colorado (Colorado’s Medicaid program) benefits that you could qualify for in certain cases*.
What are Home and Community-Based Services (HCBS)?
Home-and Community-Based Services (HCBS) are types of person-centered care delivered in the home and community.
HCBS programs address the needs of people with functional limitations who need assistance with everyday activities, such as getting dressed or bathing. HCBS are often designed to enable people to stay in their homes, rather than moving to a facility for care.
HCBS programs generally fall into two categories: health services and human services.
What are Home and Community-Based Service Waivers?
HCBS programs are often funded by state waivers. Waivers are part of the state’s Medicaid program. They provide a special group of services to a certain population.
Waivers usually require medical and financial eligibility, but state waiver eligibility requirements may not be exactly the same as state Medicaid eligibility.
Waivers We Offer
Developmental Disabilities Waiver (DD)
Provide Health First Colorado benefits in the home or community for those with developmental disabilities or delays that are most in need due to the severity of their disability.
Supported Living Services Waiver (SLS)
Provide home or community-based alternative to nursing facility care for persons who are elderly, blind, and living with a disability.
Children's Extensive Support Waiver (CES)
To provide Health First Colorado benefits in the home or community for children with developmental disabilities or delays that are most in need due to the severity of their disability.
